A lot of the wins from single payer come from the power of collective bargaining. If there is only a public option you still have the confusion over what is or isn’t covered, how is billing submitted, etc. And if there are other insurance players medical facilities can refuse to accept the government insurance. Better to take the insurance companies and let them offer supplemental coverage for people who want top tier coverage and let the government cover the basics.
That said, I think there are a number of ways to get to that point over time if a clean break isn’t politically feasible. But we need someone pushing for the more ambitious goals.

I have Medicare. Three years ago I had a heart attack requiring an ambulance ride, a couple of hours in the ER, and two nights hospitalization. My out of pocket expense was zero, zilch. I don’t know where you have gotten your “Medicare only pays 20%” talking point, but you’re mistaken.
