I too would like to know exactly what he means by this.

Leaving aside the fact that this is an incomprehensible statement, perhaps he means people with pre-existing conditions are that way because of some fault of their own.
Let’s take as an example the 1/3 of Americans who are overweight, obese, pre-diabetic, or diabetic. This is an epidemic that started just a few decades ago. There’s very good evidence that the government-recommended Food Pyramid as well as government subsidies for Big Ag, which encouraged development of low-cost, high carbohydrate foods, can be held responsible for this epidemic. So, it’s our fault for listening to our government, which is now going to toss us overboard?

That was a representative. A similar sentiment was expressed by Mick Mulvaney of the Office of Management and Budget. This must be a Republican talking points behind closed doors.
Considering how many states did not accept the Medicare help under the ACA I think the CBO can easily figure out which states will ask for waivers rather than covering people. Everyone in the house and senate should be required to buy their insurance on the open market under their new insurance plan. Then they would understand the issue.
