I don’t think the deadliest do, actually. The plague still exists–there was an outbreak in the U.S. as late as the early Twentieth Century, I believe–and killers like smallpox had to be extirpated from the world through an exhaustive immunization campaign.

The world does not owe us herd immunity. Plagues are apt to die out once they thin the herd to the point that R drops but this is not the same thing as immunity. To the extent that a plague provides immunity it does so by infecting and killing a certain percentage. With vaccination you get immunity without as many deaths.
